Medical Release Form

Before a youth can come on any trip or major event we require that we have a Medical Release Form for that person.  Beyond issues of liability, this allows the church to know about allergies, to have contact numbers, to know medical issues, and lots of other important information in case of emergency.  Please feel free to download this and return it to Jeff at the next youth event.  Take note, we ask that these are updated every year, and this next year's is right here!
